Auto Insurance (if driving for clients)
CPR Card (available onsite if you don't have already – $50 cash / $55 card)
Voided Check or Direct Deposit Form
DOEA 1 hour ADRD Certificate
DOEA 2 hour ADRD Certificate (Coming soon)
Level 2 Background Check (only needed if you have not done the screening within 4 years) Onsite available in Venice $90 cash or $95 Credit card.
For Hillsborough Applicants Only:
